Crittenden Regional Hospital Home Health and Delta Center on Aging awarded Quality Awards

Crittenden Regional Hospital’s Home Health agencies and the Delta Center on Aging were recently awarded quality awards presented during AFMC’s 17th Quality Conference. The Arkansas Foundation for Medical Care (AFMC) presented these awards to recognize their commitment to health care excellence.
The home health agencies won an Innovator Award for partnering with Crittenden Regional Hospital, UAMS, Delta Center on Aging and the hospital Foundation. Together, the group works to decrease hospitalizations from congestive heart failure. Innovator Awards are presented to home health agencies that demonstrate improved processes and outcomes. The agency must also have mentored other agencies.
“I’m delighted our team was honored again for the third year in-a-row, says Cindy Parker, RN, Director of Homecare Services, “We have a community-focused staff who are devoted to quality patient care.” Accredited by The Joint Commission since 1995, the Crittenden Regional Hospital home health agencies are the only home health agencies in Crittenden County to have earned the Joint Commission’s Gold Seal of Approval™.

The Delta Center on Aging (DCOA) was awarded a Quality Improvement Award for identifying opportunities to improve documentation within the Electronic Health
Record (EHR). This award recognizes physician offices who use either an EHR system or disease registry to capture data that measures preventive and chronic illness; to identify gaps and to subsequently implement system change; to document any barriers encountered and resolved; and to incorporate monthly documentation and progress/improvement.
The Delta Center on Aging Senior Health Clinic is an outpatient primary and specialty geriatric care facility using an interdisciplinary care model. The Center offers consultative services of healthcare providers who require expert geriatric evaluation for their patients. The DCOA Senior Health Clinic is located at 304 S. Rhodes, across from Crittenden Regional Hospital.
