What Services Are Offered?

Nurses: Registered nurses (RN) and licensed practical nurses (LPN) provide the direct skilled nursing services for the patient. Nurses may also supervise other caregivers, as required and coordinate patient care with the physician. Nurses will also train the patient and/or significant others in procedures necessary to allow the patient to remain in the privacy of his/her home.

Home Health Aides: These trained individuals, under the supervision of a registered nurse, provide services associated with the personal care of the patient When assistance with bathing, grooming, dressing, cooking, and light housekeeping are needed, home health aide services can be indispensable.

Therapists: Another important component of the home care team is the professional therapist. Physical, occupational, speech, and respiratory therapists provide essential services needed by the patient. The therapist also plays a vital role in training non-professionals to assist the patient with exercises and routine care. This can allow the patient to function in the home and recover more effectively.

Social Worker: The social worker can assist the patient and family in securing resources for meeting social, financial, and psychological needs.

How do I obtain services?

Referrals for service may be made by the patient, physicians, hospitals, family members, or friends. After receiving a referral, a Home Health staff member will call your physician to obtain an order to make an evaluation visit. A registered nurse will come to your home to determine if you qualify for home heath services. You, the patient, have the right to choose the home health agency you feel best meets your needs.
